Rowan-House provides occupational therapy-led clinical case management and rehabilitation coordination to support meaningful functional goals and communication among the people involved in a person's care.
Our approach
- Collaborative assessment of functional needs and priorities
- Individual rehabilitation planning and goal setting
- Coordination with approved providers and supports, with appropriate consent
- Progress monitoring, practical recommendations and authorized reporting
Enquiries may come from individuals, professionals, insurers, unions and VAC-related referral pathways. Services are subject to suitability, authorization and funding arrangements. Rowan-House does not guarantee VAC funding or represent itself as a designated VAC case manager.
